Apart from your username and password a telephone number is asked for as some situations can be better solved speaking with you direct. At times a simple situation can appear to become an extremely complicated one when trying to describe things in printed words!!
Remember the more information you can give the better. Error messages can often give us a good clue to what has gone wrong. In the additional information tell also us when did problem start, any changes you have made to computer etc.
